A hair transplant in Albania is a three-night trip for almost everybody: you land, you are assessed and tested, you are operated on the next morning, your scalp is checked and washed the morning after that, and you fly home on the third day. Nothing in that schedule is rushed, and nothing in it requires you to bring anyone with you. What follows is the whole thing in order, including the parts that are uncomfortable and the parts nobody warns you about, like sleeping upright for the first nights and eating with a numb forehead.

Before you fly

Once the estimate is agreed you get a date, a list of what to stop and a list of what to bring. Blood thinners, aspirin, vitamin E and fish oil stop a week before if your own doctor agrees. Alcohol stops three days before. Smoking, honestly, matters more than most people want to hear: nicotine narrows the small vessels that feed a freshly placed graft, and cutting it out for a fortnight either side of surgery is the single cheapest thing you can do for your own result.

Book the flight that lands before mid-afternoon if you can. It leaves room for the consultation and the blood tests on the same day, which frees up the morning of surgery.

Day 0: landing and the assessment

You are met at Tirana International Airport and driven in, 25 minutes off-peak, straight to the clinic or to the hotel depending on the flight. Where the clinic is and how the transfer works covers the drive, the landmarks and the route from the Durrës ferry. At the clinic: donor density measured, the pattern of loss staged, photographs from the four standard angles, blood taken for a full count and the standard infection screen. Then the hairline. It is drawn with you sitting up, awake, holding a mirror, and it gets redrawn until you and the surgeon who will treat you are looking at the same line. Bring a photograph of yourself at 25 if you have one; it helps more than a picture of a footballer.

You sleep at the hotel that night and eat normally. No shaving at home: the head is shaved in the clinic, where the length can be judged against the plan.

Day 1: the operation

Arrive around 08:00. The scalp is shaved and cleaned, the donor area is anaesthetised, and the stinging part of the whole day is over in a few minutes. Extraction takes the first two to three hours, and it is the quiet part; most patients doze, listen to something or watch a film. There is a real break with a meal in the middle, usually while the grafts are counted and sorted.

After lunch the recipient sites are opened and the grafts go in, single-hair units at the front, two, three and four-hair units behind them. A 3,000-graft case is 6 to 8 hours from start to finish, anaesthesia is local throughout, and you can ask to stop at any point. At the end the donor is dressed, you are given a head band for the swelling, a bag of medication and written instructions, and you are driven back to the hotel.

The night after, and day 2

The evening is not painful for most people, though the donor area is tender and the recipient area feels tight and hot. You sleep semi-upright at about 45 degrees for the first three to five nights, which the hotel pillows or the neck pillow you bring will decide. Swelling of the forehead is common on the second, third and fourth days, and it moves downwards before it goes.

On the morning of day 2 you come back in. The scalp is inspected graft by graft in the areas that matter, and the first wash is performed for you: lotion, soaking time, water pressure, palm only, and how to pat rather than rub. You watch and then you do it once yourself, because from the next day you are the one doing it. You leave with the wash routine in writing and a schedule for the following ten days. Day 3: flying home with a healing scalp

Nothing about a flight harms a graft. What harms a graft is a seat back, a bag, a hood or somebody’s elbow touching it. Choose a window seat, keep the head band in your hand luggage, keep the medication in the cabin, and put nothing over the recipient area other than the loose hat the clinic gives you. Aisle traffic is the real hazard on a full flight. Drink water, take the painkiller before boarding rather than after, and expect the donor area to itch.

The twelve months after you land at home

The trip ends; the case does not. Photo check-ins are scheduled at month 1, month 3, month 6, month 9 and month 12, and they are read by the surgeon who will treat you, not by a coordinator. Send the same four angles each time in daylight, which makes month-to-month comparison possible.

• Month 1: the transplanted hairs shed. This is expected, it is not failure, and it is the point at which most patients write in worried.
• Month 3: the first new growth, often fine and colourless.
• Month 6: roughly half to two thirds of the density, enough to know the direction.
• Month 9 and 12: thickening, texture normalising, the crown last.
• Month 12: the result is judged, and only then is a second session discussed if the plan always allowed for one.

Between check-ins the clinic answers messages. Swelling that gets worse after day four, a spreading redness, a fever or a graft that lifts off in the first three days are the things to message about the same day.

What to pack

A button-front shirt or two, because nothing goes over your head for a week. A loose hat, not a cap that grips the hairline. A neck pillow for sleeping at 45 degrees and for the flight. Your usual medication list with doses. Headphones, because surgery is long. Something to wear that you do not mind getting antiseptic on. Leave the hair wax, the dry shampoo and the hard-bristled brush at home; you will not use them for a month.

FAQ

Can I do it in two nights instead of three?

Yes, if your flight home leaves late on day 2 and the check and first wash are done in the morning. It is not the version the clinic recommends: the day after surgery is the day you are most tired, and an airport with a fresh donor area is a poor place to spend it. Beard and eyebrow cases are the ones where two nights genuinely works. Repair cases go the other way and usually need a fourth night.

Do I need someone to travel with me?

No. Patients travel alone every week, and the clinic arranges the pickups, the appointments and the wash. Bring someone if you would rather not be alone on the evening of surgery or if you are nervous about the flight home. A companion can sit in the theatre’s waiting area but not in the operating room.

Will I be able to work the week after?

Desk work, yes, from a few days after you land, allowing for a swollen forehead on days two to four and visible scabs until day 10 to 14. Physical work that makes you sweat, lift or wear a helmet waits until day 10 to 14, and so does the gym. If you need a letter for an employer, ask for one when you book.

What if I feel unwell in Tirana after surgery?

You have a direct number, the clinic is 25 minutes from the airport and rather less from the hotel, and someone will see you. The hotel is chosen for being close for exactly this reason. Nothing about the first 48 hours is meant to be endured quietly: a question at 21:00 on the evening of surgery is a normal question.
