What platelet-rich plasma actually is

Blood is drawn from your arm, the way it is for any blood test. The tube goes into a centrifuge, which separates it by weight: red cells at the bottom and, above them, a layer of plasma carrying most of the platelets. That layer is drawn off and injected across the thinning area in a grid of small injections. Nothing synthetic is added, and the material is used in the same appointment it was prepared in.

Platelets are the part of blood that gathers at a wound and releases growth factors, and using them on a follicle extends that idea to a structure with its own blood supply. It is worth being plain about how strong that reasoning is. The published evidence for PRP in hair loss is not settled the way the evidence for a transplant is: a transplanted follicle either grows or it does not and you can count it in a photograph, while PRP is judged on the thickness of hair that was already there.

Where PRP sits around a transplant

Around surgery, PRP is used in the recipient area where the grafts are going and in the donor area they came from. The thinking is the same in both places: a freshly placed graft has no blood supply of its own for the first few days, and the donor is left with thousands of small wounds to close.

Whether it is used in your case belongs to the surgical plan, not to a list of extras. The surgeon who will treat you makes that call, and the specialist who assessed your hairline is the one in the room from extraction to the final graft, so the person deciding is the person doing it. What PRP does not change is the calendar: transplanted hairs shed in the first weeks, growth begins around month three or four, and the result settles over the first year. No injection shortens that.

PRP on its own, before surgery is the answer

The commoner use is the simpler one. You are in your twenties or thirties, the front or the mid-scalp is thinner than it was two years ago, and your photographs show finer hair rather than bare skin. There is nothing to fill. There is hair worth keeping.

This is where PRP has something to offer, and it works best in company. Medication is the part of any keep-what-you-have plan with the strongest evidence behind it, and it is a prescription decision taken with a doctor rather than a clinic upsell: see the page on finasteride and minoxidil. It is also the situation in which a transplant is often the wrong operation for now, because a hairline drawn around hair that is still receding ages badly. How many sessions, and how far apart

There is no published course here, and be wary of a number quoted before anyone has looked at your scalp. How many sessions and how far apart depends on how much hair is still in place, how quickly you are losing it, what medication you take and what you are trying to hold on to. It is a clinical decision, made at the consultation, which is free.

What can be said in advance is the shape of the commitment, and it matters more for travel than for medicine. London is about three hours by air, Manchester about three and a quarter, Dublin about three and a half, and the clinic sits 25 minutes from Tirana airport. That is close enough for a transplant, which is one trip of three days. It is not close enough for an appointment you have to repeat, so if injections are all you need, have them where you live. What it costs and how it sits beside a package

What PRP will not do

• It does not create follicles. An area that shows skin in a photograph has lost the structures, and only grafts moved from your own donor area put hair there.
• It does not stop androgenetic hair loss. The process that thinned your hair runs underneath, which is why medication usually comes first and PRP is added to it.
• It does not replace the first two weeks after surgery. What protects new grafts is washing, sleeping position and patience: the aftercare instructions.
• It does not come with a warranty. The clinic stands behind a written guarantee for life: if transplanted grafts do not grow as planned the corrective session is covered, which does not extend to the natural thinning of hair that was never transplanted, and injections into hair that was never transplanted sit outside that.

Will PRP regrow hair I have already lost?

No. PRP is asked to support follicles that are still there and producing finer hair than they used to. Where a follicle has gone, an injection has nothing to act on. That is why the first question at a consultation is not whether you want PRP, but what your photographs actually show: thinning hair, which can be worked with, or skin, which needs grafts. The two look similar in a bathroom mirror and quite different under clinic lighting.

How many sessions will I need?

That is a clinical decision, made after your scalp is seen rather than from a web page. It depends on how much hair is still in place, how fast you are losing it and what else you are doing about it. Be careful with any clinic that sells you a fixed number of sessions and a total before it has looked at you: the number in that offer was chosen to make a price, not a plan.

Is PRP safe?

The material injected is your own blood, so there is no foreign substance to react against. What remains is the ordinary effect of injections into the scalp: tenderness for a day or so, sometimes mild swelling at the front, sometimes small bruises where the needle went in. You are asked about your medication and about bleeding disorders first, as before any minor procedure, and those answers can rule PRP out.

Is PRP included in the transplant package?

No. The package price covers the operation and the items listed beside it, and PRP is quoted separately at from €150 a session. That is deliberate. Folding it in would mean every patient paying for something only some patients benefit from, and it would make the package figure harder to compare with anyone else’s.
