Where the hair comes from, and what that costs you

Beard grafts are taken from the same safe zone at the back of the head that a scalp case draws on, which means a beard spends part of a budget you only have once. Two thousand grafts in your face are two thousand grafts your hairline cannot have later. If you are losing hair at the front as well, the order of operations is a real decision and it belongs in the analysis, not in a sales call. Hair from the back of the scalp is finer than native beard hair. It thickens somewhat in its new home but it does not become identical, so a beard built entirely from scalp hair looks slightly softer than one that grew there. Placing it densely, in the right direction, is what makes that invisible in a mirror and in a photograph.

Designing the shape before anything is placed

The beard is drawn on your face with you holding a mirror, standing up, and then checked sitting down, because a jawline looks different when the skin moves. Three rules decide the drawing.

The cheek line is set no higher than the point where your own hair naturally stops, which is usually a line from the corner of the moustache to the top of the ear. Higher than that and a beard reads as painted on.

The lower border on the neck is left soft rather than sharp, because you will shave it anyway and a hard transplanted edge cannot be moved later.

Direction changes across the face: down on the cheeks, down and inward on the chin, outward and slightly down on the moustache, where the hairs meet in the middle. A single graft placed against its neighbours’ direction is the thing people notice without knowing what they are looking at.

How many grafts each area takes

These are planning ranges to orient you. The number for your face comes from the photographs, because it depends on how much you already have.

A full beard from nothing is therefore a four-figure case and often a two-session plan, because face skin placed too densely in one sitting is where blood supply becomes the limiting factor rather than graft availability.

The two weeks people ask about

Day one, your face is dotted with tiny crusts, each one a graft. The face swells more than the scalp does and the swelling sits low, around the jaw and sometimes the lower cheeks, for two to three days. You look like you have been in a minor accident rather than a surgery.

The crusts are the visible problem. They come away between day ten and day fourteen with the washing routine you are taught on day two, and until then you do not shave, do not scratch, and keep a razor nowhere near your face.

Then the transplanted hairs shed between weeks two and six, which is the phase nobody warns beard patients about, and the face looks as it did before you came. First growth at month three or four, a real beard shape by month six, the settled result at month twelve. From that point you shave it, trim it and grow it exactly like the rest.

Can I grow a full beard if I have almost no hair on my cheeks?

Usually yes, and the limit is your donor rather than your face. A face with nothing on the cheeks takes a four-figure graft count, and the planning question is whether taking that many from the back of your head is wise given what your hairline may need in fifteen years. That is answered in the analysis with your photographs in front of it.

Will the transplanted beard hair keep growing long?

Yes. It keeps the character of scalp hair, which means it grows longer than native beard hair would and needs trimming. Most patients treat that as an advantage. The transplanted hair is permanent; hair that was not transplanted can still thin, and that applies to a beard the same way it applies to a scalp.

Can beard hair be used for my head instead?

The reverse direction is also possible and is used in repair work, where beard hair fills the mid-scalp and crown of someone whose scalp donor is spent. Beard hair is thicker and curlier, so it is not used at a hairline. If your case is a repair, that conversation starts from the previous surgery rather than from your beard.

Does it hurt more than a scalp transplant?

The anaesthetic injections in the face sting more than the ones in the scalp, and they take a few minutes. After that you feel pressure. Afterwards the face is tight and swollen rather than sore, while the donor at the back of your head is the part that aches for two or three days. Paracetamol is usually enough.

How much does it cost compared with a scalp transplant?
